I am a participating provider with several insurance plans:
If I am “in network” with your insurance plan, you will be responsible only for your co-pay. I will bill your insurance company for the remainder of my fee. If I am considered “out of network” you will pay me directly, and I will provide you with a receipt for you to submit to your insurance company for reimbursement. |
